The kickball for kids oath of conduct:

I do solemnly swear I will attend this tournament with the sole purpose of celebrating a family deserving of our support.

I will act with good sportsmanship and not embarrass myself by arguing with a referee at this family event, using language not appropriate for daytime TV, or pelting a 10-year-old with a ball in order to score an easy out.

I am assured 100% of profits from this event will benefit the family of Kickball for Kids recipients.

I recognize that if I am out of shape, I may sprain, break, or tear a muscle, ligament, or bone I never knew I had and will not hold accountable Prairie Meadow Park, the town of Westville, or anyone affiliated with this fundraiser.

As a participant in this tournament, I recognize and acknowledge that there are certain risks of physical injury and I agree to assume the full risk of any injuries, damages, or loss which I may sustain as a result of participating in all activities connected with or associated with this fundraiser.

My signature indicates I have read and fully understand that THIS IS A RELEASE. A legal guardian’s signature must accompany minors signatures.
